Role of varicocele sclerotherapy in the management of benign prostatic hyperplasia and its associated lower urinary tract symptoms (pilot study)

Abstract Background Benign prostatic hyperplasia (BPH) is inescapable with aging and can cause lower urinary tract symptoms, worsening the quality of life. New pathophysiologic mechanisms BPH development are currently under study to modulate its management. The valvular destruction spermatic veins (varicoceles development) incurs a testicular-prostatic hydrostatic pressure gradient, which conveys free undiluted testosterone prostate via backdoor passage. Subsequently, congestion androgen-dependent occur resulting in symptoms development. purpose our detect effect testicular venous sclerotherapy on enlargement related symptoms. Results Our single-arm interventional included 36 patients an age range 40–80 years. complained had prostatomegaly varicoceles by sonography. A median international symptom score (IPSS) 19 life (QoL) ranging (3–6) reflected severity Venography showed reflux all cannulated subsequent left side bilaterally 80.6% 11.1% patients, respectively. technical clinical success rates reached 91.7% 83.3%, respectively, statistically significant reduction IPSS QoL scores. We observed insignificant improvement sexual satisfaction, volumes, post-void residual PSA levels. Minimal self-limiting complications occurred overall rate 38.9%. Only 16.66% cases needed further surgery, while rest sufficient symptomatic relief post sclerotherapy. Conclusion Varicoceles be employed resolve insufficiency mitigate prostatic-related urological middle-aged elderly men, so considered confounding variable
